This is for public information that,“The Life Seva Trust” is starting a Financial Aid Scheme to give benefits to needy and poor patients to decrease their financial burden during surgery or medical management in addition to its previous program of “TLSS” (The Life Seva Scheme on last Sunday of every month).
1) The discount price should be based on price fixed for different surgeries on annual basis in April of every year
2) 20% Financial help scheme: on surgical packages and medically managed cases to the patients in Dormitory or cabin, having BPL card or declared to be poor by local MLA, Sarapancha, School Headmaster or any other prominent member of local society. A card named “FAS-20” should be supplied to them.
3) 10% Financial help scheme:on surgical packages to patients in Non AC single rooms or sharing rooms, having BPL card or declared to be poor by local MLA, Sarapancha, School Headmaster or any other prominent member of local society. A card named “FAS-10” should be supplied to them.
4) 10% Financial Medicinal helpscheme: to all patients purchasing Medicines from Pharmacy under The Life Seva Trust, irrespective of their financial status.
Patients BPL card holders or patients declared to be poor by local MLA, Sarapancha, School Headmaster or any other prominent member of local societyand getting treated at dormitory (both surgical and medical management patients) or non A/C single rooms.
Patients staying in A/C or Deluxe rooms
All the patient details should be maintained in a register, should be collected in the format of Coupon issued to each patient month wise and analyzed on a yearly basis.
